New Employee Form

In order to find the New Employee Form you will need to follow the steps provided here:


1. Start at the UIU home page (www.uiu.edu).


2. Select “IT Help Center” found at the top of the page.


3. Select "IT Forms Online" from the Links section on the right side of the screen. This will direct you to the IT Forms Online page.


4. At the IT Forms Online page sign in with your domain login credentials.


5. Once you have signed into the IT Forms Online page you will be able to click on the "New Employee Form" and fill it out with the information needed.
